Goal: The goal of this program is to improve colorectal cancer screening rates among older adults.

Impact: Participants in the intervention group had significantly higher colorectal cancer screening attendance, as well as having more positive attitudes about screening and placing a higher priority on screening.

Goal: The goal of the study was to address the special psychosocial needs of adolescents and increase contraception use, equip adolescents with the education needed to make responsible decisions related to family planning matters, and decrease unintended pregnancies.

Impact: After a one-year follow-up, teens were less likely to be pregnant. Intermediate findings at six months showed that teens in the experimental group were more likely to continue using a birth control method and less likely to experience difficulty in dealing with contraceptive-related problems.

Goal: The goal of the pilot was to assess the impact on health care use of addressing patients’ civil legal problems – the social, financial, or environmental problems that require assistance from lawyers to remedy. The lawyer was embedded in the health care team and present during case management discussions to identify specific civil legal problems and to help the team better understand how to address them. Additionally, this partnership provided civil legal aid services to patients when needed in a community health care system.

Impact: This pilot shows a medical-legal partnership for the super-utilizers of healthcare can lead to efficiencies within the health care system, reduce costs, and improve health outcomes among the most vulnerable patients.

Goal: The goal of ReachOut Central (ROC) is to improve mental health among young people through an online gaming service by teaching them the practical coping skills for dealing with major stressors in life, ranging from issues such as alcohol use to psychological distress.

Impact: The aggregate results of this statistical analysis point to ReachOut Central's potential to impact and improve certain factors, such as coping ability, but also to relatively unexplored gender-dependent outcomes for other factors like alcohol consumption.

Goal: The goal of the Fresh Food Financing Initiative is to remove financing obstacles and operating barriers for supermarkets in poor communities, stimulate investment of private capital in low-wealth communities, create jobs, prepare and retain a qualified workforce, and reduce diet-related diseases by providing healthy food.

Goal: The purpose of the Task Force was to:

1. identify and explore current and proposed strategies for
- building incomes and assets for working families;
- promoting financial education and literacy; and
- protecting working families from predatory and abusive financial services;

2. maximize the coordination of programs that affect working families and create an environment in which they can work together with greater effectiveness and impact;

3. work with charitable foundations, local government agencies, advocacy groups, community and religious leaders, academia, policy and research organizations, and other entities as may be appropriate, to carry out the purpose of the Task Force.

Goal: The goal of this study was to reduce pediatric asthma-related symptoms by installing central heating in homes.

Impact: Central heating successfully improves home heating, dampness, and energy efficiency. Through home modifications, asthma-related symptoms (nocturnal cough and days lost from school) can be reduced among children.
