Goal: Insite’s mission is to provide a safe environment for people to inject drugs and thus reduce injecting activity in public while linking drug users to health care services such as primary care, addiction counseling and treatment.

Impact: Opening supervised injected facilities have resulted in significant reductions in public injection drug use related issues and increase in referrals to social services and detoxification programs in Vancouver.

Goal: The result of hundreds of community meetings and thousands of volunteer hours, The Green Book describes the three legs, or goals, of sustainability that guide development at Stapleton:

- Environmental Responsibility: Demonstrate the economic and community benefits of a long-term commitment to reducing consumption of natural resources and impacts on the natural environment.

Goal: The goal of this program is to provide parents with the necessary skills to improve their parent/child communication and overall family functioning.

Impact: STEP has been implemented in more than 1,000 schools, agencies, churches, and mental health treatment facilities since 1976, reaching more than 4 million parents. Outside the US, STEP has been implemented in Australia, Canada, Germany, Ireland, Mexico, New Zealand, the Philippines, Romania, and South Korea.

Goal: To use tai chi exercise to improve balance and decrease incidence of falls among older adults.

Impact: The program shows that ta chi can significantly improve health-related outcome measures in older adults and such a program can be practically and effectively implemented and maintained in community settings.

Goal: The goal of the Teen Health Project is to provide adolescents with the skills necessary to prevent HIV risk behaviors.

Impact: The Teen Health Project shows that community-level interventions that include skills training and engage adolescents in neighborhood-based HIV prevention activities can produce and maintain reductions in sexual risk behavior, including delaying sexual debut and increasing condom use.

Goal: To facilitate behavior change among overweight adults that leads to sustained weight loss.

Impact: Those who use Text4Diet are exposed to customized advice regarding healthy behavior changes. Participants on average saw an average 6-pound weight loss during a four-month use period. This is a statistically significant difference from weight loss in the usual care comparison group.

Goal: Text4Health aims to improve immunization rates in urban, underserved, low-income populations via text messaging.

Goal: The goal of this intervention is to reduce high-risk behavior among African American youth as measured by student self-reports of violence, provocative behavior, school delinquency, substance use, and sexual behaviors (intercourse and condom use).

Impact: AAYP reduced rates of risky behaviors among male African American youth.
