The ultimate goal is to support the systematic adoption, implementation, and evaluation of successful programs, practices, and policy changes. The database provides carefully reviewed, documented, and ranked practices that range from good ideas to evidence-based practices.

The goal of this program is to improve colorectal cancer screening rates among older adults.
Participants in the intervention group had significantly higher colorectal cancer screening attendance, as well as having more positive attitudes about screening and placing a higher priority on screening.

Stepping On aims to reduce the prevalence of falls among older adults. Falls are not a normal part of aging and the risk and/or prevalence of falls may be decreased by taking certain steps, including vision checks, medication management, strength & balance exercises, and home safety checks. Falls and/or the fear of falling often lead to a loss of independence.
This evidence-based program demonstrated a 31% reduction in falls among participants in an Australian trial. It reached a 50% reduction in falls among participants in a U.S. workshop.

HeartStrings has reached over 3,200 individuals with disabilities, long-term illness, and assisted-living needs, as well as aging adults with dementia since its start in 2006.

To improve children's nutritional status, increase their activity level, enhance their self-esteem, and create life-long health habits by using a multidisciplinary, community- and family-based system approach, and by engaging local health care professionals with community agencies.
The Fit Kids/Fit Families program shows that multidisciplinary, community- and family-based approaches to children's exercise, weight, & nutrition can have an effect on healthier nutritional choices, increased physical activity, decreased sedentary activity, healthier behaviors, and BMI reductions.
